NOTICE: ALL RENEWALS SUBMITTED AFTER NOVEMBER 30, 2011, WILL BE SUBJECT TO A LATE PENALTY FEE OF $25.00 PER MONTH OR ANY PORTION OF A MONTH BEGINNING DECEMBER 1, 2011. THE OPTION TO RENEW LATE EXPIRES AT 5:00 P.M. (CDT) JUNE 30, 2012.
